***Full-time Position based in the Family Practice After Hours Clinic located in Hattiesburg, MS***

Job Description Summary:

 As a NavCare Medical Assistant you will engage with prospective and established clients by placing and receiving calls to discuss Chronic Care Management services, confirming consent into the program. You will also respond to patient needs such as ensuring medications are refilled, scheduling appointments, obtaining records from other providers and performing monthly telephonic wellness assessments as support for the Wellness Team.

Essential Job Functions/Responsibilities:

A. Receives, obtains consent, and processes client data. Answers incoming calls and directs to appropriate personnel, applying superior customer service skills.

B. Understands the Medicare Benefit for Chronic Care Management (CCM) and Transitional Care Management (TMC) and articulates to the customer/patient the benefits of the program and the impact to their continued wellness plan.

C. Enters all information accurately and promptly into electronic comprehensive care plan according to NavCare processes and procedures.

D. Works collaboratively with physicians’ office staff and is seen as part of their team when assigned in a physician office.

E. Follows all privacy policies and maintain the confidentiality of protected healthcare information (PHI).

F. Provides general office support.

G. Will perform other duties as assigned.

Position Qualifications:

1. Certification through American Association of Medical Assistants (AAMA) or National Association of Health Professionals (NAHP) is required. Applicants must provide their Certification ID Number.
2. Excellent customer service skills.
3. 2 years’ experience in a physician’s office, hospital or similar setting.
4. Bilingual/Spanish-speaking highly desirable.
5. Strong data entry and verbal/written communication skills are essential.
6. Experience with a variety clinical systems and ability to quickly master new IT systems.
7. Proficient in Microsoft Office applications and data entry.
8. Adaptability to work with various disciplines in multiple locations (i.e. NAVCARE center, physician office, facility).
9. Flexibility to handle a workload that fluctuates greatly based on needs.
