Job Description Summary:



The Manager, Care Management Services is responsible for leadership, staff development, coaching, developing tactical direction and providing clinical support for our 24 hour/7 days-a-week Care Center. Manages all onsite and remote clinical staff. Leads a cost-effective, high-quality and compliant clinical operations team with a strong commitment to our employees, clients and the patients we serve. Participates in division-wide process improvement initiatives which reinforce our commitment to developing and empowering our clinical staff.





Essential Job Functions/Responsibilities:



A. Directs the overall operations of NavCare through the management of performance objectives and metrics to ensure that performance objectives, service delivery and performance commitments are consistently achieved.

B. Builds appropriate operational plan to lead and respond to business requirements.

C. Uses performance data to manage Care Center operations; works in strong collaboration with NavCare leadership to continuously assess opportunities for improvement and optimization of resources

D. Performs monthly clinical record audits to ensure that clinical staff complies with Medicare requirements and the organization’s quality standards.

E. Responsible for customer satisfaction, achieving operational and financial targets aimed and meeting production standards.

F. Develops direct reports through coaching in leadership, employee management, continuous operational improvement and staff development. Promotes a work place culture that values and inspires quality, innovation, integrity and unparalleled clinical support services.

G. Maintains knowledge of and ensures compliance with all State and Federal Laws governing Medicare Chronic Care Management (CCM) and Transition Care Management (TCM) and follows best-practice guidelines and legislative standards

H. Functions as a key resource regarding CCM and TCM. Provides comprehensive training on all aspects of NavCare to patients, families, physicians and staff. Educates team members on policy changes and new program offerings

I. Assumes the role of Chronic Care Management RN as needed.



Position Requirements:



1. Associates or Bachelor’s degree in Nursing from an accredited institution.

2. Licensed Registered Nurse (RN) required. License must be active and unencumbered in order to apply, obtain and retain nursing licenses in multiple states.

3. 5 years’ experience in nursing leadership providing oversite to onsite staff. Oversite of remote staff experience is highly desirable.

4. Experience with a variety electronic health records (EHR or electronic medical records (EMR) systems and the ability to quickly master new IT systems; proficient in Microsoft Office applications.

5. Ability to provide leadership across multiple locations with a track record of leading teams focused on delivering high quality clinical service.

6. Exceptional leadership/management skills to include excellent oral, written and presentation skills.

7. Strong problem solving and critical thinking skills.

8. Occasional overnight travel is required.
