Job Description Summary:



The Implementation Specialist is responsible for ensuring the successful on-boarding of new providers and facilities as they implement Chronic Care Management and Transitional Care Management Programs. On-boarding is the most important phase of the client lifecycle, and the Implementation Specialist is responsible for building and maintaining the client’s satisfaction. This position will schedule and coordinate key activities with internal departments, external resources and the client to ensure a complete implementation while maintaining client satisfaction. The Implementation Specialist will be required to travel to multiple locations nationwide to implement new clients and provide onsite enrollment support.



Essential Job Functions/Responsibilities:



A. Responsible for complete project implementation and the resources necessary to ensure a smooth implementation. Works with all internal resources to coordinate complete implementation including ensuring connectivity is in place prior to implementation.

B. Develops implementation plan appropriate for each client and follows implementation process. Develops and maintains implementation documentation and communicates status and concerns to team.

C. Manages multiple implementation tasks and projects simultaneously. Develops and maintain professional client relationship and manage expectations with respect to timeline and enrollment expectations.

D. Oversees ongoing project/status meetings and communication with clients to ensure that the implementation project is moving forward and in accordance with the implementation process.

E. Coordinates resolution of client issues and concerns as it relates to implementation.

F. Ensures that an effective training program relative to client’s electronic medical record is in place and training completed with internal teams.

G. Keeps up-to-date on platform releases and new features.

Works with clients to review medical records and identify patients that meet the criteria for admission into the CCM/TCM program. Enters all information accurately and promptly into proprietary software platform.

H. Works collaboratively with physicians’ office staff and is seen as part of their team during the on-boarding process.

I. Understands the benefit of Chronic Care Management (CCM) and Transitional Care Management (TCM) and can articulately explain the benefits of the programs to clients.

J. Continues to look for opportunities and make recommendations to improve the overall implementation experience for internal and external clients.

K. Follows all privacy policies and maintain the confidentiality of protected healthcare information (PHI).



Position Qualifications:



1. Registered Nurse or Licensed Practical Nurse highly desirable.

2. Two (2) years of experience in a healthcare setting, preferably in a medical office management/supervisor role which includes physician and provider interaction/provider relations.

3. Experience with a variety electronic health records or electronic medical records systems and the ability to quickly master new IT systems.

4. Exceptional customer relationship building skills, along with strong data entry and verbal/written communication skills, are essential.

5. Proficient in Microsoft Office applications (Excel, Power Point, Word, etc.).

6. High degree of flexibility and ability to travel and work in a variety of settings (i.e. Care Center, physician office, facility, or home office). Up to 90% travel is required on a regular basis.

7. Highly organized and ability to multi-task, meet deadlines and work in a fast-paced environment.
