The Chronic Care Management (CCM) Float Pool LPN/LVN provides clinical expertise to patients with chronic illness via telephonic outreach to support and maintain the patient’s health and wellbeing. Assist patients with chronic illness(es) in the development and achievement of health goals as directed by physicians. This is a floater position that allows for flexibility in scheduling and accepting assignments. Work assignments may be completed remotely, at a provider’s office, facility and/or in a NavCare Call Center according to business needs.


A. Collaborates with physician, patient and other health care providers to guide patients to better health outcomes using evidence-based care management and care coordination resources in an effort to reduce unnecessary hospitalizations and emergent care visits.
B. Documents all communication and coordination of patient contact in electronic documentation system including aggregation and clinical summaries. Assures documentation includes tracking and time-stamping to support CCM and TCM billing.
C. Ensures the electronic comprehensive care plans and care summaries are created and maintained accurately per Medicare Regulations and organizational policies and procedures.
D. Ensures that care is provided in accordance with Federal and State guidelines and complies with accepted professional standards, principles and licensure regulations.
E. Assists with complaint resolution, risk management issues and guidance procedures as needed.
F. Follow all privacy policies and maintain the confidentiality of protected healthcare information (PHI).


1. Licensed Practical/Vocational Nurse with current licensure in assigned state(s). Compact license preferred. License must be unencumbered and must have the ability to apply for nursing licensure in multiple states.
2. Experience in a call center, triage position, consulting environment or similar health care environment.
3. Bilingual/Spanish-speaking highly desirable.
4. Minimum of three (3) years’ experience in caring for or providing case management of chronically ill patients.
5. Risk stratification and population health experience are a plus.
6. Superior customer service skills.
7. Demonstrates excellent observation, verbal and written communication skills.
8. Experience with clinical systems and comfortable learning new IT systems.
9. Proficient in Microsoft Office applications.
10. Must be able to work in a seated position for extended periods of time.
