Nurse Case Manager (RN Registered Nurse) Job Responsibilities As a Nurse Case Manager, you will direct and supervise all personnel involved in patient care within a specific geographic area. This involves making initial patient evaluation visits and consulting with the attending physician to establish and update the plan of care. You will be responsible for ensuring quality patient care and the allocation of agency resources. Additional responsibilities: • Assigning patients to Nurses and other members of the health team in conjunction with the Clinical Manager • Consulting with the Clinical Manager about problems concerning the admission of patients to Agency care and coordination of services • Assisting Clinical Manager in the supervision of Nurses and Home Health Aides • Reviewing plans for patient treatment and notifying team members of any changes • Participating in the nurse on-call system • Participating in staff development activities, meetings, and training sessions • Initiating and conducting Care Team Conferences for patients • Completing assessments, re-assessments, and resumption of care and providing related supervision in a timely manner • Reviewing medical records for assigned case load and completing discharges • Ensuring that all records are in compliance with agency policies and procedures • Following all privacy policies and maintaining the confidentiality of protected healthcare information (PHI) Job Requirements Our patients need your nursing skill, compassion, and dedication to quality! Nurse Case Manager requirements: • Licensed registered nurse in the State served by the Agency • Current CPR certification • Bachelor’s degree from a program approved by the National League for Nursing for Public Health preparation, preferred • 3+ years of experience as a professional nurse with at least 1 year of experience in the field of community health, preferred • Reliable transportation, a valid driver’s license, and current automobile liability insurance • Clinical supervision experience, preferred Nurse Case Manager (RN Registered Nurse) Benefits At US CareNet, we care about our employees as much as we care about our patients.
